Hypertension Awareness & Prevention Project (HAPp)
The Hypertension Awareness and Prevention Program is a self-monitoring program that will equip participants with a FREE blood pressure monitor and other helpful items.
The program lasts four months, from March through June.
Each month participants are expected to measure their blood pressure at home two times and also meet with the Extension Agent two times to discuss blood pressure readings.
There will also be a nutrition education workshop occurring once per month that participants are highly encouraged to attend.
This is a FREE program.
During this program you will:
- Learn how to monitor your blood pressure accurately at home.
- Increase your knowledge about hypertension and healthy living.
- Become an advocate for your health
- Learn how to make connections between lifestyle and blood pressure reading
In order to participate:
- MUST BE age 18 or older
- MUST HAVE hypertension diagnosis or on antihypertensive prescription
- MUST HAVE interest in and readiness for the program
- NO significant cardiac event within the last 6 months
- NO atrial fibrillation or other arrhythmias
- NO lymphedema
